# Anorectal Abscess

An infection with pus near the anus

## What it is

An anorectal abscess is an acute infection in which a collection of pus forms in the tissues around the anus or rectum, almost always from an obstructed, infected anal gland. It causes intense, constant, throbbing pain that worsens when sitting or having a bowel movement, along with swelling, redness and, in some cases, fever and malaise. It is an emergency: treatment is surgical drainage, and delay increases the risk of the infection spreading. About half of abscesses progress to an anal fistula, so follow-up afterward is essential. ## Causes and risk factors

- Obstruction and infection of the anal glands
- An infected anal fissure
- Crohn's disease
- Weakened immunity or diabetes
- Previous anal trauma or procedures

## How it is diagnosed

In most cases, the coloproctologist makes the diagnosis through a physical exam: a history of intense, worsening pain associated with a tender, warm, reddened bulge in the perianal area is characteristic. Deeper abscesses, with no visible skin signs but intense pain and fever, may require a CT scan or pelvic MRI to locate the collection. Blood tests help assess the severity of the infection in patients with diabetes, weakened immunity, or signs of a generalized infection.

## How we treat it at Femi Saúde

Treatment is surgical drainage, performed as soon as possible under local anesthesia, sedation or regional anesthesia, depending on the size and location of the abscess. The cavity is opened and emptied, and the wound is left open to heal from the inside out. Antibiotics are reserved for cases with extensive cellulitis, immunosuppression, diabetes or systemic signs, and do not replace drainage. After drainage, the coloproctology team at Femi Saúde recommends sitz baths, dressings and pain relief, and reassesses the patient a few weeks later to identify and treat a fistula, should one form.

## When to see a gynecologist

See a coloproctologist if you feel intense, constant anal pain that worsens when sitting, especially with swelling, redness or a warm lump near the anus, and if there is fever or malaise. An abscess does not get better on its own and should not be treated with antibiotics or ointments alone. People with diabetes or compromised immunity should seek care even more quickly.
