## What it is

Breast cysts are fluid-filled sacs that form in the breast ducts, very common between ages 35 and 50 and influenced by the hormonal changes of the cycle. They can be single or multiple, small and unnoticeable, or large enough to form a soft, tender lump that tends to increase before menstruation. Ultrasound easily confirms the diagnosis, showing the fluid content, and classifies the cyst as simple — benign, requiring no treatment — or complex, which deserves investigation. Simple cysts that are painful or bothersome can be drained by fine-needle aspiration, during the consultation itself, guided by ultrasound. Periodic follow-up is sufficient in most cases.

## Most common symptoms

- Soft or elastic lump with well-defined borders, in one or both breasts
- Local pain or tenderness that increases before menstruation
- Increase in size before menstruation, with relief afterward
- A feeling of heaviness or discomfort in the breast
- Small cysts that only show up on ultrasound
- There can be several cysts at the same time

## Causes and risk factors

- Hormonal variations of the menstrual cycle
- Age between 35 and 50, in the phase leading up to menopause
- Fibrocystic changes of the breasts
- Hormone therapy at menopause, which can keep cysts active
- Blockage of a milk duct with fluid buildup

## How it is diagnosed

On clinical exam, your breast specialist identifies a soft, mobile lump with well-defined borders, often tender to the touch. Ultrasound, performed on site, confirms the diagnosis by showing the fluid content and classifies the cyst as simple — which is benign — or complex, with thick walls or solid content, which deserves investigation. Mammography is ordered depending on age. In complex cysts or those that refill, ultrasound-guided aspiration allows the fluid to be analyzed or a biopsy to be performed.
