# Type 2 Diabetes

## What it is

Type 2 diabetes is a chronic disease in which the body doesn't use insulin well and, over time, produces less of the hormone than it needs, keeping blood glucose elevated. It develops slowly, often without symptoms, which is why many people discover the disease on routine tests. Excess weight, a sedentary lifestyle and family history are the main risk factors. Left uncontrolled, high glucose damages blood vessels, kidneys, eyes, nerves and the heart. With an individualized plan combining diet, physical activity, medication and regular follow-up, it is possible to keep glucose at safe levels and live well while preventing complications.

- Intense thirst and dry mouth
- Frequent urge to urinate, including at night
- Excessive hunger or unexplained weight loss
- Fatigue and blurred vision
- Wounds that are slow to heal
- Recurrent urinary or vaginal infections
- Tingling in the feet and hands

## Causes and risk factors

- Insulin resistance associated with excess abdominal weight
- Family history of diabetes
- A sedentary lifestyle and a diet high in sugar and ultra-processed foods
- Gestational diabetes or polyendocrine metabolic ovarian syndrome (SOMP)
- Age over 45 and prolonged corticosteroid use

## How it is diagnosed

Diagnosis is made with blood tests: fasting glucose of 126 mg/dL or higher, HbA1c of 6.5% or higher, or glucose of 200 mg/dL or higher two hours after a glucose load, confirmed with a second test when there are no clear symptoms. The visit includes measuring blood pressure, weight and waist circumference, as well as cholesterol, kidney function and urine protein tests. The endocrinologist also orders an eye exam and checks the feet, to identify early any complication already present at the time of diagnosis.

## How we treat it at Femi Saúde

Treatment is built in stages and reviewed at every visit. The foundation is lifestyle change: a balanced diet, regular physical activity, adequate sleep and weight loss when needed. Medications are chosen according to glucose levels, weight, heart and kidney health, and the patient's preferences, and some classes offer benefits beyond glucose control. Insulin may be needed at specific stages. HbA1c is measured every three months until the target is reached, and the endocrinology team at Femi Saúde also monitors blood pressure, cholesterol and screening tests for complications.

## When to see a gynecologist

See your endocrinologist if you have excessive thirst, urinate frequently, feel tired or have blurred vision, or if a test showed elevated glucose or HbA1c. It's also important to see one if there is diabetes in your family, if you had gestational diabetes, if you are overweight with altered blood pressure or cholesterol, or if you already have the diagnosis and want to review your treatment and targets.
