# Chronic Pelvic Pain

## What it is
Chronic pelvic pain is pain in the lower abdomen or pelvis that lasts six months or more, either continuously or in cycles. It can have several origins — gynecological, such as endometriosis, adenomyosis, adhesions and cysts, but also urinary, intestinal, muscular and emotional — and often more than one cause is involved at the same time. For this reason, the investigation is thorough, including a detailed clinical history, physical examination and imaging tests and, in some cases, laparoscopy. Treatment is directed at the cause and at managing the pain, and may combine medication, hormonal therapies, pelvic physical therapy, surgery when indicated and multidisciplinary follow-up, always with attentive listening and support.
## Most common symptoms
- Pain in the lower abdomen for six months or more
- Pain that worsens during menstruation or through the cycle
- Pain during intercourse
- Pain when urinating or having a bowel movement
- A feeling of heaviness or pressure in the pelvis
- Fatigue and changes in sleep and mood
- Pain that limits work and daily activities

- Urinary causes, such as interstitial cystitis and overactive bladder
- Irritable bowel syndrome and other intestinal causes
- Pelvic floor muscle tension and emotional factors
## How it is diagnosed
The consultation begins with a detailed history: where the pain is located, when it started, how it varies through the cycle, what makes it worse or better, and how it affects daily life. The physical exam assesses the abdomen, pelvis and pelvic floor muscles. Transvaginal ultrasound and, depending on the case, MRI investigate endometriosis, adenomyosis, fibroids and cysts; urine and laboratory tests rule out urinary and infectious causes. When the tests do not clarify the picture, laparoscopy allows the pelvis to be visualized and any findings to be treated.
## How we treat it at Femi Saúde
Treatment is directed at the cause and at controlling the pain, and often combines more than one approach. The Femi Saúde gynecology team may recommend painkillers and anti-inflammatory drugs, medications for nerve-related pain, and hormonal therapies, such as contraceptives, progestins or the hormonal IUD, when the pain is linked to the cycle. Pelvic physical therapy addresses muscle tension, and multidisciplinary follow-up, with a psychologist, nutritionist and other specialists, is part of the care. When there is endometriosis, adhesions or cysts that justify intervention, laparoscopy may be indicated. The plan is individualized, built together with the patient, and reviewed throughout follow-up.
