# Hyperthyroidism

When the thyroid produces too much hormone

## What it is

Hyperthyroidism occurs when the thyroid releases too much hormone and speeds up the functioning of the entire body. The patient may notice a racing heart, weight loss despite eating well, tremors, feeling hot, anxiety and insomnia — symptoms that are sometimes attributed to stress alone. The most common cause is Graves' disease, of autoimmune origin, but nodules that produce hormone autonomously and inflammation of the gland can also cause the condition. Left untreated, excess hormone strains the heart and weakens the bones. With proper diagnosis and follow-up from the endocrinologist, it is possible to control hormone production and choose, together with the patient, the strategy best suited to her case.

## Most common symptoms

- Palpitations and a racing heart at rest
- Weight loss despite eating normally
- Hand tremors and restlessness
- Excessive heat intolerance and increased sweating
- Anxiety, irritability and insomnia
- More frequent bowel movements than usual
- Bulging or irritated eyes (in Graves' disease)

## Causes and risk factors

- Graves' disease (autoimmune)
- Thyroid nodules that produce hormone autonomously
- Thyroiditis, including the initial phase of postpartum thyroiditis
- Excess iodine or thyroid hormone from tablets
- Use of medications such as amiodarone

## How it is diagnosed

The workup begins with a clinical history and physical exam, assessing heart rate, thyroid size, tremors and eye signs. On blood tests, TSH is suppressed while free T4 and T3 are elevated. TRAb antibody testing confirms Graves' disease, while Doppler ultrasound shows the gland's blood flow and any nodules. When it's necessary to distinguish between causes, a thyroid scintigraphy shows whether the whole gland or just one nodule is producing excess hormone. An electrocardiogram may be ordered if there is arrhythmia.

## How we treat it at Femi Saúde

There are three treatment options, and the choice depends on the cause, age, desire to become pregnant and the patient's preference: antithyroid medications, which reduce hormone production and are usually the first choice; radioactive iodine, which lowers the gland's activity; and surgery, reserved for large goiters or specific cases. Beta-blockers relieve palpitations and tremors while hormone levels are still high. During treatment, the endocrinology team at Femi Saúde monitors lab tests every few weeks, adjusts doses and tracks any side effects, while also guiding eye and bone care.

## When to see a gynecologist

See your endocrinologist if you've noticed a racing heart for no reason, weight loss without dieting, tremors, excessive heat intolerance and sweating, or sudden-onset insomnia or anxiety, or if you've noticed swelling in your neck or changes in your eyes. Also see one if a test showed low TSH, if there is thyroid disease in your family, or if symptoms appeared in the months after giving birth.
