# Recurrent Pregnancy Loss

## What it is
Recurrent pregnancy loss, or recurrent miscarriage, is defined as two or more pregnancy losses before 20 weeks. Each loss is a difficult experience, and its repetition brings anxiety and many questions. Known causes include genetic abnormalities of the embryo — the most common —, uterine abnormalities such as a septum, fibroids and polyps, hormonal disorders such as thyroid disease and uncontrolled diabetes, thrombophilias and antiphospholipid syndrome, as well as factors such as age, smoking and obesity. In about half of cases, even after a complete investigation, the cause is not identified, and even so the chances of a successful pregnancy remain high. The evaluation includes the couple's history, blood tests, karyotyping, ultrasound and hysteroscopy when indicated. Treatment is directed at whatever is found, with close, supportive follow-up in the next pregnancy.
## Most common symptoms
- Two or more pregnancy losses before 20 weeks
- Repeated losses in the first trimester, with or without a visible embryo
- A later loss, after 12 weeks, even if only one
- Bleeding and cramping early in pregnancy
- Intense anxiety and fear when planning another pregnancy
- History of blood clots, autoimmune diseases or thyroid disorders
## Causes and risk factors
- Genetic abnormalities of the embryo, the most common cause
- Uterine abnormalities, such as a septum, fibroids, polyps and adhesions
- Hormonal disorders, such as thyroid disease and uncontrolled diabetes
- Antiphospholipid syndrome and thrombophilias
- Advanced maternal age, smoking, obesity and alcohol use
- No identified cause, even after a complete investigation
## How it is diagnosed
Investigation is usually recommended after two losses and involves the couple. During the consultation, your gynecologist reviews how each loss occurred, the gestational age, previous test results and the general health of both partners. Testing includes hormone and thyroid level tests, screening for antiphospholipid syndrome and other thrombophilias, karyotyping of the couple, and ultrasound to evaluate the uterus; hysteroscopy is indicated when a septum, polyps or adhesions are suspected. When possible, genetic analysis of the pregnancy tissue helps guide the next steps.

When no cause is identified, close follow-up in the next pregnancy, with more frequent visits and ultrasounds, is the main approach. Emotional support and psychological care are offered to the couple.
