# Bacterial Vaginosis

## What it is

Bacterial vaginosis is the most common cause of vaginal discharge in women of reproductive age. It occurs when there is an imbalance in the vaginal flora: protective lactobacilli decrease and other bacteria, such as Gardnerella, multiply. The typical symptom is a thin, grayish-white discharge with an unpleasant, fish-like odor, which worsens after intercourse and during menstruation; itching and burning are less common. It is not considered an STI, but it is associated with new partners, vaginal douching and the use of irritating products. Without treatment, it increases the risk of other infections, complications in gynecological procedures and problems during pregnancy. Diagnosis is made in the office, through examination and analysis of the discharge. Treatment uses oral or vaginal antibiotics, and preventing recurrences — which are frequent — involves proper hygiene habits, avoiding douching and, in some cases, maintenance regimens and probiotics.

## Most common symptoms

- Thin, uniform, grayish-white discharge
- Unpleasant, fish-like odor
- Odor that worsens after intercourse and during menstruation
- Constant feeling of moisture in the genital area
- Mild itching or burning, less common
- Symptoms returning after treatment

## Causes and risk factors

- Imbalance of the vaginal flora, with a drop in lactobacilli
- Vaginal douching and use of soaps or irritating products
- A new partner or multiple sexual partners
- Intercourse without a condom
- Smoking

## How it is diagnosed

Diagnosis is made in the office. Your gynecologist listens to your description of the discharge, the odor and the factors that worsen your symptoms, and performs a gynecological exam, noting the characteristics of the discharge. Analysis of the vaginal discharge, including pH measurement and microscopic examination, helps confirm vaginosis and distinguish it from candidiasis, trichomoniasis and other causes of discharge. When necessary, additional laboratory tests and screening for sexually transmitted infections may be ordered.

Treatment for bacterial vaginosis uses antibiotics, taken orally or applied vaginally, depending on the condition and the patient's preference, with guidance on the duration and when to avoid intercourse. Since recurrences are common, Femi Saúde's gynecology team reviews the habits that disrupt the flora, such as douching and irritating products, and provides guidance on proper intimate hygiene. In recurring cases, maintenance regimens and probiotics may be proposed, depending on the case. Treating the partner is not usually necessary, but it may be considered in specific situations. The plan is individualized and adjusted as it progresses.

## When to see a gynecologist

Book a consultation if discharge with odor persists for more than a few days, returns after treatment, or is accompanied by pelvic pain, fever or bleeding outside your period. Pregnant women and those about to undergo gynecological procedures should treat vaginosis beforehand, so it's best not to delay the assessment.
