Fibrocystic Changes of the Breast

Tender, irregular breasts before menstruation

What it is

Fibrocystic change is the most common benign condition of the breast and affects more than half of women at some point between ages 30 and 50. It is not a disease, but a response of the breast tissue to the hormonal fluctuations of the cycle: the breasts become denser, with hardened, irregular or nodular areas and small cysts, tender and swollen in the week before menstruation, improving afterward. Assessment is important to differentiate these areas from true lumps, with a clinical exam and ultrasound. Treatment is aimed at relieving symptoms: guidance on bra fit, diet and caffeine, pain relievers or anti-inflammatories during painful periods and, in selected cases, hormonal medication. Periodic follow-up keeps things reassuring.

Most common symptoms

  • Tender, swollen breasts in the week before menstruation
  • Hardened, irregular or granular areas to the touch
  • Small lumps that appear and disappear with the cycle
  • A feeling of heaviness and increased breast fullness
  • Improvement of symptoms after menstruation
  • Slight nipple discharge with compression, in some cases
  • Symptoms more intense in the upper outer part of the breasts

Causes and risk factors

  • Response of breast tissue to hormonal fluctuations of the cycle
  • Age between 30 and 50, with improvement after menopause
  • Hormone therapy at menopause, which can keep symptoms active
  • Family history of benign breast changes
  • Excess caffeine and an ill-fitting bra, which increase discomfort

How it is diagnosed

The diagnosis is based on the history of symptoms, which worsen before menstruation and improve afterward, and on a clinical exam of the breasts, in which your breast specialist recognizes the irregular, granular or hardened areas typical of fibrocystic change. Ultrasound, performed on site, distinguishes these areas from solid lumps and shows the small cysts; mammography is added depending on age. When a finding does not have the usual appearance, aspiration or ultrasound-guided biopsy clarifies the doubt.

How we treat it at Femi Saúde

Since it is not a disease, care focuses on relieving symptoms. The breast health team at Femi Saúde recommends helpful habits, such as a well-supporting bra, reducing caffeine and salt before menstruation, and regular physical activity, along with pain relievers or anti-inflammatories on painful days. In selected cases with intense symptoms, hormonal medications may be considered for a limited time. Painful cysts can be drained by ultrasound-guided aspiration at the clinic. Periodic follow-up, with clinical exams and imaging, allows the irregular areas to be observed over time. The plan is individualized according to symptoms and life stage.

When to see a gynecologist

Tender, irregular breasts before menstruation are common, but a lump that persists after the period, grows, or stands out from the surrounding tissue deserves evaluation. Also see your breast specialist if there is spontaneous nipple discharge, skin retraction or pain at a fixed point. Book a consultation to clarify the doubt.
